New York State Workers Compensation Board requires us to obtain proof of workers compensation and disability from either the contractor or property owner before we can issue a building permit.
1. Homeowners who physically live in the home where work is being done and plan on doing their own work can complete a waiver from worker compensation. This form is available in our office and must be notarized.
2. Contractors and/or property owners who do not carry workers compensation and/or disability please go to the link above for the the workers compensation website. Once at the website, scroll down on left hand side for form CE-200. Please remember that on the form you must have the following: applying for building permit from Town of Southport, address of the property where work is being done and estimated cost of project. Once complete, print out the form and bring to our office. If needed, we have a computer and printer in our office for your use. Workers Compensation Board on Lake Street can also assist you.
